A genuine emergency still gets through — a site that runs out of cement mid-pour on a Saturday must be able to buy. But the reason is typed, stored against the order and printed on it, so an exception is visible in the record instead of being indistinguishable from authorised work. A chain with no escape hatch just gets worked around outside the system, where you cannot see it at all.
